2023 Word of Life Youth Camp: June 11-16
FOLLOW THE DIRECTIONS BELOW TO PARTICIPATE:
1. Fill out the digital WORD OF LIFE PARTICIPATION AGREEMENT
2. Fill out the digital SALEM MEDICAL CONSENT Form
3. PAY HERE - SELECT WOL Youth Camp Payments from the list. (Total is $397 - Deposit $75 by March 23, 2023)
4. (Optional) Print, Sign, and Return these forms to Holly Akins:
GO KART RELEASE
SCHOLARSHIP APPLICATION
5. (Optional) Print PACKING LIST
6. (Optional) Print INFO PACKET
7. Parents will have an opportunity to supply additional funds for special activities, travel dining, and special camp options at Salem WOL Youth meetings.
8. Once Salem registers our group, parents will receive a courtesy email to fill out a CAMPER LIABILITY WAIVER per child from the WORD OF LIFE Camp facility. This form is required by WOL and should be completed for each participant.
